Case Management is defined by the Case Management Society of America as a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
Case Management services include:
- Case management for catastrophic and complex care cases
- Client centred approach
- Liasson with care providers, adjusters, lawyers and rehabilitation team members to ensure quality and timely service
- Knowlegable in relevent legislation (Accident Benefits/LTD)
- Completion of appropriate Case management or Team Reports
Life Care Planning assessment and intervention services include:
- Life Care Planning and reports for medical/ legal purposes completed by a Canadian Certified Life Care Planner
- Future Cost of Care assessments related to ongoing medical, rehabilitation and personal care needs resulting from persistent and residual impairments
- Life planning (from current into future) for clients with life changing impairments and disabilities
- Professional team support (OT, PT, Case Management, RSW and Vocational Rehab Consulting) for rebuilding life goals and plans following complex and catastrophic impairments
- Completions of various medical legal assessments for identifying levels of function and disability
