  • Meet legal and professional responsibilities under Ontario law

How We Protect Your Information

Your information is protected using secure electronic records, limited staff access, and privacy training. We only share information when required for your care or by law. We only share information with your consent or in some cases when working with your circle of care. Your circle of care includes other health care practitioners who are on your health care team (physiotherapist, occupational therapist, family practitioner, nurse practitioner, specialists).

You Have a Right to Access Your Health Records and / or Request a Correction

Under Ontario’s Personal Health Information Protection Act (PHIPA), individuals have the right to access and request corrections to their personal health information held by a therapy home care company, which is considered a health information custodian. You may ask to see or receive a copy of your health record by contacting the company directly, typically by making a written request, although verbal requests may also be accepted. If you believe any information in your record is inaccurate or incomplete, you may submit a request for correction, and the organization is required to respond within the timelines set out in PHIPA. If a correction is not made, you have the right to have a statement of disagreement added to your record.
